An internal alert surfaces here when a clerk's recent activity trips the guardrail-override threshold (3 events in the last 50 rows / 30 days) or sits at more than 2× the cross-clerk negative median with at least 3 such events. Use the trigger codes A01 and A02 when escalating to the clerk's chain of command — escalation is human-driven.
